The Kansas High School Activities Association (KSHSAA) requires all students participating in athletic activities, including cheer and dance, to have current physical and concussion forms on file in the Athletic Office before a student can participate in any athletic/spirit practice or competition.
Physicals must be completed and dated on or after May 1, 2018 to be current for the 2018-2019 school year.
Parents need to complete the first page of the physical form and sign and date all areas in yellow. Parents also need to sign and date the KSHSAA Eligibility Checklist and the Concussion form.
Once the Doctor's exam has been completed, please complete all paperwork and turn it in to the Athletic Office. The Athletic Office is open year-round, so you do not need to wait until the season begins to turn it in.
The final deadlines for completed and signed pages of the physical and concussion forms is the first day of practice:
Fall - August 13, 2018
Winter - November 12, 2018
Spring - February 25, 2019
But again, you don't have to wait until the final deadline!
Physicals may be scanned and emailed to email@example.com,
mailed in or dropped off in the Athletic Office.
