The goal of our athletic care is to keep our student athletes on the field as safely as possible throughout their career. CJ is available after school, before, during, and after practices and is in attendance at all home sporting events, freshman through varsity. 

Physical Form

The Kansas High School Activities Association (KSHSAA) requires all students participating in athletic activities, including cheer and dance, to have current physical and concussion forms on file in the athletic office before a student can participate in any athletic/spirit practice or competition.

Physicals must be completed and dated on or after May 1, 2023, to be current for the upcoming school year. Parents need to complete the first page of the physical form. Page four of the physical and the last page of the concussion forms need to be signed by both the student-athlete and the parent.

CJ Hess joined Performance Rehab in 2020 after finishing his Athletic Training degree at the University of Saint Mary. During his time with Performance Rehab he has had the opportunity to work with numerous youth club sports in the Kansas City area as well as professional soccer. His favorite sport to cover and watch is hockey. CJ was a collegiate Track and Field athlete at Baker University competing in the javelin and hammer throw, so he understands how much it means for athletes to stay on the field and continue to play.
