Our Policy
Sports Physicals are required for all Kansas State High School Activities Association (KSHSAA) sports, cheer and dance.
Physicals must be dated on or after May 1, 2025 and are good for the entire 2025-2026 school year.
For the 2025-26 school year, we are utilizing Privit, a digital health and safety communication platform for student sport physicals.
All physicals and concussion forms must be completed and on file in Privit by the following deadlines:
Fall – August 11, 2025
Winter – November 10, 2025
Spring – February 23, 2026

Physical Form
The Kansas High School Activities Association (KSHSAA) requires all students participating in athletic activities, including cheer and dance, to have current physical and concussion forms on file in the athletic office before a student can participate in any athletic/spirit practice or competition.
Physicals must be completed and dated on or after May 1 to be current for the upcoming school year. Parents need to complete the first page of the physical form. Page four of the physical and the last page of the concussion forms need to be signed by both the student-athlete and the parent.
